I acknowledge that I am participating in the Hills Night Hockey Association (HNHA) All Abilities Come and Try Experience voluntarily.

I understand that participation in hockey and associated activities may involve inherent risks, including but not limited to falls, collisions, contact with equipment or other participants, and other injuries that may occur during sporting activities.

I agree to participate responsibly, follow the instructions of HNHA coaches, officials and volunteers, and advise an HNHA representative of any condition, injury, limitation or other circumstance that may affect my ability to safely participate.

Medical and Personal Insurance

I acknowledge and agree that I am responsible for my own medical, hospital, ambulance, personal accident and/or other insurance arrangements associated with my participation in the event.

HNHA does not provide personal medical insurance for participants unless specifically stated otherwise.

I understand that I am responsible for any medical, hospital, ambulance, rehabilitation or other expenses that may arise from an injury, illness or medical condition occurring before, during or after my participation, to the extent permitted by law.

Emergency Medical Treatment

In the event of an emergency, I authorise HNHA representatives to seek appropriate medical assistance for me where reasonably necessary if I am unable to provide consent at the time.

I understand that any costs associated with medical treatment or emergency services are my responsibility, subject to any applicable insurance or legal rights.

Release and Waiver

To the extent permitted by law, I acknowledge the risks associated with participating in the All Abilities Come and Try Experience and agree that HNHA, its committee members, coaches, officials, volunteers and representatives will not be responsible for loss, damage, injury or expense arising from my participation, except where liability cannot lawfully be excluded or limited.

Nothing in this waiver is intended to exclude, restrict or modify any rights or protections that cannot legally be excluded under applicable Australian consumer or other legislation.
